
Become A Patient
To receive medical, dental and/or social services at San José Clinic, low-income, uninsured individuals and families must first receive an eligibility interview. Those that qualify for San José Clinic services will then receive a Clinic card, providing them access to Clinic services.
Eligibility interviews are conducted on a walk-in basis from 8 a.m. to 3 p.m. Monday through Friday. No appointment is necessary.
You must bring identification, proof of income and proof of address to your eligibility interview. Examples of these are outlined below.
1) IDENTIFICATION
Bring one form of identification for each individual within your family that is applying for Clinic services. Examples are listed below.
Adult:
-Texas Driver's License
-Texas Identification Card (current or expired)
-Passport or Visa
-U.S. Immigration documents
-ID from other country
Child:
-Birth Certificate
-Birth Fact Record
Note: A Social Security card is not required; however please bring one for each family member that has one.
2) PROOF OF INCOME
Bring all of the following documents that apply to you.
-One month worth of recent paycheck stubs (if this is not available please bring a completed wage verification letter.)
-Welfare benefit documents (TANF and Food Stamps)
-SSI or Social Security certification documents
-Unemployment documentation
-Child Support
-Most recent tax return
3) PROOF OF ADDRESS
Bring Documentation showing where you have lived for the past month.
Examples: Lease, Medicaid document, house payment,
voter registration, utility bill, or 2 pieces of valid mail in your name with current address
